The P0491 code indicates that the secondary air injection (SAI) system is not flowing enough air into the exhaust on bank 1 (the side of the engine with cylinder 1). The SAI system pumps fresh air into the exhaust manifold during cold starts to help the catalytic converter heat up faster and burn off excess hydrocarbons and carbon monoxide during the warmup period. It's purely an emissions-related system.

The most common failure pattern starts with the one-way check valve. When this valve fails, hot exhaust gases flow backward into the air injection pump, which can melt or damage the pump's internal components. This is why many mechanics recommend replacing both the check valve and the pump together — if the valve has been leaking, the pump has likely been damaged by the backflow. Cracked vacuum hoses are another common cause and are much cheaper to fix.

While this code won't affect drivability under normal warm driving conditions, you may notice a rougher idle during cold starts and you'll fail an emissions test. If you hear a whining or grinding noise from the air pump area, the pump itself has likely failed. On some vehicles (particularly certain BMW, Volkswagen, and Toyota models), carbon buildup in the air injection ports is a known issue that requires removing the intake manifold or cylinder head to clean.
